Care Coordinator Team
Our Care Coordinator team, generously funded by Traverse Independence, supports individuals experiencing homelessness across our shelter sites and in the community. They manage a caseload focused on guests with traumatic brain injury (TBI), substance use, mental health, justice, medical, and developmental needs, working in partnership with our Housing Stability Workers to address both health and housing goals equally. Recognizing that people experiencing homelessness often face barriers to equitable healthcare, the team ensures participants receive the services and resources they need to achieve improved health, stability, and long-term housing success.
