
Housing Stability Programs for Individuals and Families Experiencing Homelessness
If you’re experiencing homelessness and struggling to find market rent units, reliable transportation to appointments, or someone to advocate to a landlord on your behalf, our Housing Stability Team supports a caseload of individuals and families from the By-Name List. These individuals and families have completed the common assessment tool and been prioritized for this support.
What Is Housing Stability?
Our Housing Stability Team supports a caseload of individuals and families from the By-Name List. These individuals and families have completed the common assessment tools and been prioritized for this support.
Housing Stability is a recovery-oriented approach to ending homelessness that centers on quickly moving people experiencing homelessness into independent and permanent housing and then providing additional supports and services as needed.

Social Worker
The Social Worker also works to foster a sense of belonging and connection to the community, liaises with community health partners, and informs risk and care planning. Additionally, they join the Outreach Team in the community, connecting with individuals in encampments to map out health supports or pathways to shelter and ensure access to necessary services, promoting mental well-being and long-term housing stability.

Grace Gardens Permanent Supportive Housing
The focus of the Grace Gardens Permanent Supportive Housing initiative is to support individuals that have experienced chronic homelessness. It has been proven that people who have experienced chronic homelessness benefit from housing with 24/7 support services. These individuals often also experience mental and physical health issues and severe addiction.
