Housing Focused Outreach
Our Housing Focused Outreach worker visits locations known to have individuals who are precariously housed, at risk of homelessness, or experiencing homelessness to actively engage and build relationships and create a case management plan with the individual based on their needs.
What Is Housing Focused Outreach?
Working alongside our Housing Stability team to end homelessness in the community, our Outreach worker assesses the needs of individuals and families through a standardized assessment tool, assists them to acquire housing, identifies intervention recommendations and supports their transition to a case manager.
What is Rough Sleeping?
Rough Sleeping is the practice by homeless individuals that are sleeping without adequate shelter, typically on the streets and in city parks. While the number of those sleeping rough only accounts for a fraction of the total number of individuals experiencing homelessness in our community, these individuals are some of the most vulnerable.
Many people rough sleeping are not aware of the support and resources available to them. Our Housing Focused Outreach worker is continually working to connect with these individuals.
How Can You Help Individuals Who Are Rough Sleeping?
If you come across an individual in Guelph-Wellington rough sleeping, you can help by calling the Guelph’s Bylaw office at
519-837-2529.
Following your call, Guelph Bylaw will contact Stepping Stone. Stepping Stone will then arrange for the Housing Focused Outreach Worker to visit the person at their site and assess the need for support.
This protocol helps Stepping Stone keep an accurate By-Name List of individuals experiencing homelessness in Guelph-Wellington.
What Is The By-Name List?
The By-Name list is a tool used to help identify and get to know the names of every individual in Guelph-Wellington suffering from chronic or acute homelessness.
Once added to the By-Name List, our team engages and fosters relationships with the individuals to create a case management plan based on their needs.
These plans help to guide the individuals into a long-term housing solution and to track our progression towards eliminating homelessness in Guelph-Wellington.
